Office and Financial Policies

Appointments

While we try to schedule appointments at your convenience, please understand that not all appointments can be scheduled after school. Treatment visits for all ages and exams for young children are scheduled in the morning, when children are more rested and our office is able to take additional time if needed. Dental visits are considered an excused absence and we will provide you with a note for school.

Missed Appointments

Please call us in advance if you are unable to keep your scheduled dental appointment. We understand that life circumstances occasionally cause dental appointments to be missed. As such, we will reschedule the first missed dental appointment for established patients. However, after three missed dental appointments, we will terminate our professional relationship with you, so you may find a dental office that better fits your time scheduling needs.

Dental Insurance

As a courtesy, our office will file claims with your dental insurance provider and help answer questions about your coverage. You will be responsible for the estimated amount not covered by your insurance at the time of service. Most insurance plans do not pay 100 % of the fees charged and have a deductible or co-pay which must be met before any insurance benefits can be received. Please understand that insurance companies only provide dental offices with an estimate, and your total out of pocket expense may differ from the estimate they initially provide to our office. You are responsible for understanding and knowing your dental insurance coverage. Your dental insurance policy is a contract between you and your insurance carrier. Our dental office, CDOA, is not a party to your contract, so please know that after 90 days, you will owe to CDOA any amounts not paid by your insurance company.

Finances

We accept cash, Visa, American Express and Master card. We also accept checks with proper identification. The patient’s parent or guardian is responsible for payment of all services rendered on behalf of their child or dependent. Please note that if parents are divorced, the parent or guardian who signs the office paperwork will be considered the financially responsible party. CDOA does not get involved in divorce disputes. If other payment arrangements are needed, please speak to our front desk prior to your appointment.

Forms

Some of the initial forms needed by our office are available on line. Please print, fill out and bring these forms to your appointment.
